Mild Cognitive Impairment: Symptoms, Diagnosis, and What Comes Next

Mild cognitive impairment (MCI) causes noticeable memory and thinking changes — but it's not Alzheimer's. Learn what MCI means, how it's diagnosed, and what you can do if you think you might be living with it.

If you’ve noticed that your memory or thinking isn’t quite what it used to be, you may be experiencing something called mild cognitive impairment, or MCI. It’s more common than most people realize, and importantly, it is not the same thing as Alzheimer’s disease.

Here’s what the research says, what experts recommend, and what living with an MCI diagnosis actually looks like.

What Is mild cognitive impairment?

Mild cognitive impairment is a condition in which a person experiences noticeable changes in memory or thinking that go beyond what’s typical for their age — but that aren’t severe enough to interfere with their ability to live and function independently.

MCI is not a disease like Alzheimer’s or other forms of dementia. It’s a clinical description of a person’s cognitive status, determined through cognitive testing, medical history review, and other evaluation methods.

MCI can affect memory specifically — this is called amnestic MCI — or it may affect other brain functions such as language, attention, spatial navigation, or decision-making. Not everyone with MCI will go on to develop dementia, and sometimes the root causes of MCI are treatable. 

About 10 to 20 percent of cases of MCI do develop into dementia within a year if their underlying condition goes untreated.

What are the symptoms of mild cognitive impairment?

The symptoms of MCI can be subtle, which is part of why the condition is widely underdiagnosed. Research suggests MCI is significantly underdetected in adults over 65. 

People with MCI may have symptoms that include include:

Memory lapses, such as forgetting conversations, appointments, or recently learned information more frequently than before.

Language difficulties, including struggling to find the right word or follow the thread of a conversation.

Attention and focus problems that make it harder to concentrate or multitask.

Impaired judgment or decision-making ability, including difficulty weighing options or solving everyday problems.

Sleep disturbances, like changes in sleep quality that may also affect daytime cognitive performance.

Fluctuations in hormones, particularly in women during perimenopause and menopause. These have been linked to cognitive changes that may resemble or contribute to MCI.

Depressive symptoms or mood shifts can both mimic and co-occur with cognitive decline.

How is MCI different from Alzheimer’s disease?

MCI and early-stage Alzheimer’s disease share similar symptoms, including brain fog and memory difficulties, but they have meaningfully different causes and outlooks.

Alzheimer’s disease is a progressive neurodegenerative disorder caused by specific disease processes in the brain — most notably the buildup of amyloid plaques and tau tangles. These changes lead to memory loss, confusion, and changes in behavior. MCI, by contrast, may stem from a wide range of health conditions, some of which are treatable or reversible.

Research suggests that approximately one in four cases may be reversible, depending on the underlying cause. While there’s no cure for Alzheimer’s, there are two FDA-approved monoclonal antibody drugs which may slow disease progression.

What causes MCI?

While there isn’t one specific cause of developing MCI, several factors are associated with a higher likelihood of developing it. Age is the strongest risk factor. The likelihood of developing MCI increases significantly after age 65.

Some people with insomnia experience MCI since sleep deprivation prevents the brain from recharging. Working with a doctor to treat insomnia and improve your sleep quality can help reverse this form of MCI.

Long COVID symptoms, such as brain fog, can overlap with MCI and the possible connection is being researched. Traumatic brain injury and stroke can damage specific parts of the brain, leading to cognitive and memory problems.

Cardiovascular health plays a large role in brain health. High blood pressure, high cholesterol, diabetes, and obesity are all associated with increased MCI risk.

People who carry the ApoE4 gene variant, a known genetic risk factor for Alzheimer’s disease, have a higher likelihood of developing MCI and of progressing from MCI to Alzheimer’s.

How is MCI diagnosed?

MCI diagnosis typically involves a combination of evaluations. Many people who experience MCI wonder if their symptoms are simply a result of normal aging, but it’s important to consult medical professionals like a neurologist to make that distinction.

According to neuropsychologist Emily Paolillo, PhD, assistant professor at the UCSF Memory and Aging Center, the key distinction in diagnosis is severity. 

“This term [MCI] is really used to characterize a mild level of cognitive impairment,” she said. “Typically, we have some criteria to determine what is mild versus what’s more severe. At the more severe stage, we would call that dementia,” whereas, “MCI is really defined by a mild level of cognitive impairment where you can still function independently.”

Clinicians will often ask people about their ability to handle the “instrumental activities of daily living,” such as managing finances and medications, being able to shop independently, preparing meals, and completing household chores. “If there’s evidence of cognitive impairment, but you can still do all of those things independently,” Paolillo said, “the person’s cognitive difficulties are characterized at this level of mild cognitive impairment.”

At a more severe level, the same pattern of symptoms would qualify as dementia.

A standard MCI evaluation may begin with a medical history review to take a detailed look at both the patient’s personal health history and family history of cognitive conditions. 

From there, the physician typically conducts cognitive testing through structured assessments that measure memory, attention, language, and other cognitive functions. You might also be asked to do a physical assessment, including a blood test, to rule out other conditions that could be causing cognitive symptoms, such as thyroid disorders, vitamin deficiencies, or medication side effects. 

Emerging diagnostic tools, including blood-based biomarker tests that detect Alzheimer’s-related proteins, are increasingly being explored for MCI evaluation, though they are not the gold-standard in diagnosing MCI since its causes can be very complex.

Physicians may also order MRI or CT scans to identify structural abnormalities in the brain, like the presence of tau or clear signs of traumatic brain injury, that could explain cognitive changes.

What are the treatment and management options for MCI?

Currently, no medications are FDA-approved for MCI, but there are ways to help manage the symptoms. Management depends largely on what’s driving the cognitive changes. 

Some people with MCI return to normal cognition. Others remain stable for years. Some do progress to dementia, including Alzheimer’s disease. Regular monitoring through annual cognitive assessments is helpful in assessing progression and any evolving symptoms. 

The three main areas of management are:

Medication: If an underlying condition is contributing to cognitive symptoms (such as depression, anxiety, sleep apnea, or a thyroid disorder), treating that condition through medication may improve or stabilize cognition. In some cases, doctors may consider medications for Alzheimer’s.

Lifestyle changes: Regular physical exercise, a healthy diet, cognitive stimulation, and social engagement have been shown to delay the onset of cognitive decline and slow down its progression.

Managing cardiovascular and metabolic health conditions: Controlling hypertension, blood sugar, diabetes, and cholesterol may help reduce the risk of cognitive decline progression and limit their impact on cognitive symptoms in MCI.

How can you live well with MCI?

One of the hardest parts of an MCI diagnosis is the uncertainty. The condition sits in a gray zone — it may improve, stay the same, or progress — and there’s often no way to know which path lies ahead.

Retired psychotherapist Leah Fisher describes the emotional challenge of living with MCI as “learning how to be calm in the face of uncertainty. There’s so much not knowing with MCI… Will it progress quite slowly? Which has been the case for me, thank goodness, so slowly that I can be thinking ahead, preparing even, being kind of funny about it. Or is it going to slide fast into a diagnosis none of us would like to have?”

Fisher, who has no prior songwriting experience, co-wrote a song, “If I Don’t Forget,” with Laura Fannon as a way to bring levity to her experience. 

Wally Cox was in his early 60s when he noticed subtle lapses in memory, and was subsequently diagnosed with MCI. A year later, his condition had progressed to Alzheimer’s. Despite this, Cox is living well with dementia and takes an active role as an advocate and leader in peer support groups for others navigating this journey.

“I spend a lot of time in our groups talking particularly to people who’ve been recently diagnosed, [saying] that this isn’t the end of your life, it’s just another chapter,” he said. “There’s still a lot to do. I can still garden, I can still ride a bike…and love my children.”

Cox said his diagnosis has given him time to strengthen his faith and his relationships with his wife, family and friends. “So despite the horror, it’s been a blessing,” he said. “I have two choices. I can give up or I can get out, and I decided to get out.”
